2. Request a copy over the phone
3. Come to North Cobb High School to pick up the notarized certificate in the Main Office

WORK PErmit
You will need to go to the DOL website and follow the instructions to start a new work permit.

After you have completed that, you will print a copy of the Work Permit Data Sheet and give it to the employer. The employer will need to complete the employer portion using the link that is provided on the sheet. Once this has been completed, you will then bring the Data Sheet to the school, and the main office will complete the work permit process.
